About the role:
This position is responsible for providing initial assessment and goal directed care planning for adult clients and carers who are referred for Social Support and/or Respite Services at Sunbury and Cobaw Community Health.

Clients live in the local community and may be referred via My Aged Care or may be participants of the National Disability Insurance Scheme (NDIS) or be holders of Home Care Packages.   Assessments are generally conducted in the home of a client.
