Cobaw Community Health and Sunbury Community Health will formally merge on 1 January 2021 and will be known as Sunbury and Cobaw Community Health.

For those accessing Cobaw services and programs, very little will change. Clients will continue to receive the same high level of care from the same location.

All client records at Cobaw Community Health will be transferred to Sunbury Community Health. Current and past clients of Cobaw Community Health may make a request for a copy or written summary of their health record to be sent to them or their nominated health service provider. These requests must be received by Friday 20 November 2020 and will incur a fee of $15 per file.

From 1 January 2021, client health records will be transferred to Sunbury Community Health at 12-28 Macedon Street, Sunbury VIC 3429.

What will change?

Cobaw staff will continue to work from the same sites and with the same clients. The amalgamation of Cobaw Community Health and Sunbury Community Health requires us to let you know that from 1 January 2021, your client file will be owned by Sunbury Community Health Centre Ltd (SCH). What will the new organisation’s approach be to managing my information?

The new organisation will work to the same privacy principles as Cobaw currently does. For more information please refer to the current SCH Privacy policy and SCH Privacy and Confidentiality procedure which aligns with Cobaw’s current compliant approach.

Why are these policies branded as Sunbury Community Health and not Sunbury and Cobaw Community Health?

The new entity Sunbury and Cobaw Community Health will commence on 1 January 2021. The existing policies of Cobaw Community Health and Sunbury Community Health will be merged and rebranded to Sunbury and Cobaw Community Health next year.  However our legal requirements and our current compliant approach will remain unchanged.
